Disclosure Statement and Agreement for Internet Account Access

By submitting a completed Home Banking sign-up form, you acknowledge all terms of the Electronic Funds Transfer Disclosure, and accept the terms and conditions of The Chicago Patrolmen's Federal Credit Union Home Banking agreement.

ELECTRONIC FUNDS TRANSFER DISCLOSURE STATEMENT This Agreement and Disclosure (the "Agreement") constitutes the Agreement between you and The Chicago Patrolmen's Federal Credit Union with respect to the provision of Internet Account Access (Home Banking) Webloan and Statements Online (the "Service") and contains the disclosures required by the Electronic Funds Transfer Act. This Agreement applies only to transactions initiated by Online Access and/or Online Billpayer Services. In this agreement, "I", "you" and "your" refer to the member and any joint owner or authorized user of the member account(s). "We", "us", "CPFCU", ''our'', and "credit union" refer to The Chicago Patrolmen's Federal Credit Union. The "Service", "CPFCU Home Banking" and "Internet Account Access'' refers to CPFCU's Online Access. Please read this disclosure in its entirety since it contains disclosures which affect you. PLEASE PRINT this document for your future reference. All electronic funds transfer services provided by The Chicago Patrolmen's Federal Credit Union in relation to its accounts, as defined in the Electronic Funds Transfer Act, are subject to the following terms and conditions. Your use of our electronic funds transfer services, by means of Internet Account Access, constitutes your agreement to be bound to these terms and conditions.

Account Access: Any account owner who has the Personal Identification Number/Password (PIN) or any person to whom you or your joint owner have given the PIN is an authorized user of your accounts and can conduct any transaction on your accounts that you can conduct yourself. You are responsible for all transactions authorized users make, whether or not they stay within the scope of permission you give them to use your accounts. The only way to stop an authorized user from accessing your accounts is for you to go online and choose another PIN known only to you or contact the credit union to deactivate Online Access on your account. Your electronic access will automatically be deactivated if you do not use the service for a period of (60) days. To reactivate the service, you will need to enroll using the application and have a PIN mailed to you.

General Information About the Service: You understand that in order to enroll in the Service, you must have a CPFCU account in good standing and have no outstanding past-due loans or judgements with the credit union. You may use CPFCU Home Banking virtually any time, day or night, 7 days a week. However, CPFCU Home Banking may be temporarily unavailable due to record updating, software and hardware upgrades and technical difficulties. In addition, access to CPFCU's home banking is made available pursuant to a license agreement by and between the Credit Union, USERS, Digital Insight, and FIS. Any interruption of service or access caused by USERS, Digital Insight, or FIS will also prevent your use of the Service.

Equipment and Technical Requirements: I understand that to use CPFCU Home Banking I must have a personal computer (PC), a PIN number, Internet access through an Internet Service Provider and Internet Browser Software. I understand that the Credit Union does not make any warranties on equipment, hardware, software, or Internet Provider service, or any part of them expressed or implied, including, without limitation, any warranties of merchantability of fitness for a particular purpose. The Credit Union is not responsible for any loss, injury or damages, whether direct, indirect, special or consequential caused by the Internet Service Provider, CPFCU Home Banking, or any related software used in the installation, use or maintenance of your personal computer hardware, software, or other equipment.

Types of Services/Transfers Available: Personal Finance will allow you to do the following: Balance Inquiries on all your savings and checking accounts Transfers funds between savings and checking accounts within your available limits Transfer funds to make loan payments (with the exception of Mastercard payments) Withdrawal by check - mailed to you. (Payable to the Primary Member) Apply stop payments to checks (Fees apply to each check stopped) Check inquiry - use to verify a check has cleared your account Review your account history by month for up to six months Make advances on Overdraft Line of Credit I may designate accounts at the Credit Union between which I may transfer funds electronically through Internet Account Access. All of these accounts must be in my name. I understand that under Federal Reserve Board regulation D, I am permitted to make no more than six (6) transfers or withdrawals from a savings suffix per month using CPFCU Audio Teller, Overdraft Transfers, and CPFCU Home Banking. (Transfers or withdrawals from a checking account are not REG D transactions). I understand that Chicago Patrolmen's Federal Credit Union complies with Regulation D. Stop Payments may be entered via CPFCU Home Banking. You are allowed to enter a single draft number or a range of draft numbers on the Stop Payment page of CPFCU Home Banking. Each draft, or range of drafts that is stopped requires a thirty dollar ($30.00) fee to be assessed to the Member's account. By logging onto CPFCU Home Banking, you are verifying your approval for a stop payment and the assessed fee, as if it were your signature on a stop payment form. The limit on your amount to transfer is the available balance in your account(s). I authorize the Credit Union to transfer funds electronically between my designated account(s) according to my instructions initiated through CPFCU Home Banking.

Member Liability Disclosure: Notify us at once if you believe your Personal Identification Number (PIN) has been lost or stolen. Telephoning is the best way to keep your possible losses down. If you fail to notify us promptly you risk loss of all the money in your account (plus your maximum overdraft line of credit). If you tell us within 2 business days, you can lose no more than $50 if someone used your PIN without your permission. If you do NOT tell us within 2 business days after you learn of the the loss or theft of your PIN, and we can prove we could have stopped someone from using your PIN without your permission if you had told us, you could lose as much as $500. Also, if your statement shows transactions that you did not make, notify us at once. If you do not notify us within sixty (60) days after the statement was mailed to you, you may not get back any money you lost after the sixty (60) days if we can prove that we could have stopped someone from taking the funds if you had notified us in time. If you believe that your PIN has been lost, stolen, or copied; or that someone has transferred or may transfer funds from your account(s) without your permission, call or notify us in writing at:

NOTE: Knowledge of your PIN will allow access to your account(s) by unauthorized persons. Memorize your PIN. If it is necessary to write it down, keep it in a completely secure place for your protection.

Business Hours: The Chicago Patrolmen's Federal Credit Union is open Monday through Friday 8:00AM to 5:00PM and Saturday 9:00AM to NOON, central standard time, excluding all Federal Holidays and as posted.

Documentation of Transfers: You will receive a confirmation screen with reference information after every transfer you make which you may print for your records. Your monthly statement will show all electronic fund transfers. The CPFCU Home Banking transfers are coded as "personal credit union" transactions. Any documentation provided to you, which indicates that an Electronic Funds Transfer has been made, shall be admissible as evidence and proof that such a transfer was made.

Credit Union Liability for Failure to Make Transfers: If we do not complete a transfer to or from your account(s) on time or in the correct amount according to our agreement with you, we will be liable for your losses or damages, which you prove, are directly caused by our action or failure to act. However, there are some exceptions to our liability. We will NOT be liable for instance: If through no fault of ours, you do not have enough funds in your account(s) to make the transfer, If the transfer would go over the available line of credit limit on your account or other available pre-authorized line of credit If we are legally restricted from transferring funds to or from your account(s) If circumstances beyond our control (such as flood or fire) prevent the transfer, despite reasonable precaution that we have taken There may be other exceptions not listed above, but stated in our agreement with you. Notwithstanding anything contained herein to the contrary, the credit union will under no circumstances be liable for consequential damages.

Account Information Disclosure: We will disclose information to third parties about your account or the transfers you make: where it is necessary for completing transactions in order to verify the existence and condition of your account for a third party, such as a credit bureau or a merchant to comply with government agency or court orders, etc. if you give us written permission.

In the event you discover an error on your statement or record of an electronic funds transfer, you must notify us of the error within sixty (60) days after we sent the FIRST statement on which the problem or error appeared. When you call or write to us in this regard, you should: 1. Tell us your name, account number, and telephone number 2. Describe the error or the transfer you are unsure about, and explain as clearly as you can why you believe it is an error or why you are placing a request for more information. 3. Tell us the dollar amount of the suspected error. If you tell us orally, we may require that you send us your complaint or question in writing within ten (10) business days. We will determine whether an error occurred within ten (10) business days after we hear from you and will correct any error promptly. If we need more time, however, we may take up to forty-five (45) days to investigate your complaint or question. If we decide to do this, we will credit your account within ten (10) business days (twenty (20) business days if the transfer involved a new account) for the amount you think is in error, so that you will have the use of the money during the time it takes to complete our investigation. If we asked you to put your complaint or question in writing and we do not receive it within 10 business days, we may not credit your account. Your account is considered a new account for the first thirty (30) days after the first deposit is made, unless each of you already has an established account with us before this account is opened. We will tell you the results within three (3) business days after completing our investigation. If we decide that there was no error, we will send you a written explanation. You may ask for copies of the documents used in our investigation.

Amendments: We may amend all terms and conditions at any time, however we will give you a notice of twenty-one (21) days before the amendment becomes effective if the amendment will result in increased charges or liability to you, fewer types of available electronic funds transfers, or limitations of the transfers you make. Such notice will be mailed to you at the last address of record on our files. If, however, an immediate change in terms and conditions is necessary for security reasons, we may amend these terms and conditions without prior notice. We reserve the right to levy service fees in accordance with fee schedules adopted by the Credit Union from time to time. If other terms and conditions are in conflict with this disclosure, this disclosure statement for CPFCU Home Banking will prevail.
